Jan will then contact you personally, so please make sure you include contact details. You will be asked to complete a form and provide some financial information. You can download the forms below or talk to Jan first and then be sent them via email or post.

These forms are to help the Trustees deal with your situation. All the information you give is treated confidentially and your personal information is always anonymised before it is shared with the Trustees. 

Trustees consider applications as they are received and can provide support quickly to those in need. They also meet every three months to review all ongoing cases and give additional consideration to every application. 

Applicants for support are also asked to agree to and sign our ‘working together’ document  which explains what we can do and what we expect from those applying to Iprovision. Click here to download the document . You can also download here our data protection  and safeguarding policies  .
